The Link-me+ model supports GPs to provide tailored mental health care for adults experiencing common mental health issues such as low mood, worry and stress.
Link-me+ is a model of mental health care developed through extensive consultation with patients and clinicians within a research environment over the last 20 years. The University of Melbourne and Eastern Melbourne Primary Health Network have partnered to implement the Link-me+ Mental Health Model of Care in primary care. The model supports GPs in providing tailored mental health care according to patients’ needs and the areas the patient would like to target to improve their mental health. Through Eastern Melbourne Primary Health Network (EMPHN), GPs will also have access to additional support, such as academic detailing sessions and access to a service directory. Link-me+ supports GPs to optimise mental health care for their patients whilst empowering patients to be active participants in their care.
Why Link-me+
Link-me+ is a Model of Care built on 20 years of research. It is a model of care that helps GPs to provide targeted support to patients experiencing the most common mental health challenges of low mood, worry and stress.
A Randomised Controlled Trial (RCT) of the model involving 23 general practices in Primary Health Networks across three states showed improvements in mental health and better outcomes for patients with patients with low mood, worry and stress.
How does it work?
The Link-me+ Model of Care helps to provide tailored treatment recommendations for people experiencing the most common mental health challenges of low mood, worry and stress.
An important part of the Link-me+ is the Link-me+ questionnaire, which is completed by patients 18 years old and over. The questionnaire results provide helpful information to guide decisions about treatment or support recommendations.
